Family Caregiver Support
We provide knowledge and information for caregivers to help them make educated decisions and reslove problems in their caregiver role, whether in the planning or advanced stage of care. The following services can help the caregiver keep a loved one at home longer, and delay or prevent placement in a nursing home or assisted-living facility:
- Referrals and assistance for services from local community-based providers
- Respite to relieve caregivers temporarily from their responsibilities
- Services to complement existing care
- Lending library of books and videos on care-giving issues
Case Assistance
We help older adults and their families to determine eligibility and apply for benefits and services.
Cafe’ Style Meals
This program provides hot noon meals at various dining cafe’s in the county for adults age 60 and older and their spouses regardless of age. There are no income-eligibility requirements. Call 518.843.1478 or 518.673.5048 at least one day before to reserve a meal.
Cafe’s are open Monday through Friday at 11:30 am and serve at noon. Other sites are open one day per week. Special evening meals with entertainment are offered at various cafe’s which open at 4 p.m. and serve at 5 p.m. Reservations should be made by the Wednesday prior to the evening meal. Call 518.843.2300 for a schedule. Registration, including a nutritional risk assessment, is required annually for all participants.

Friendly Visitor
Weekly visits are provided by volunteers for companionship with home-bound elderly.
Home Energy Assistance Program (HEAP)
Provides fuel or utility credit with energy suppliers for people who rent or own. HEAP also provides other heat-related emergency benefits and furnace repair help for adults age 60 and older, and for those under age 60 and disabled.
Health Insurance, Counseling and Assistance Program (HIICAP)
We have information about Medicare, Medicaid, Medigap, managed care, Medicare D and EPIC prescription coverage, and claims assistance. By appointment only.
Expanded In-home Services For The Elderly (EISEP)
Provides help at home with bathing, dressing, shopping, laundry and cleaning. This cost-sharing program is based on monthly income and a sliding fee scale. If you are eligible for Medicaid, this service is available from the Department of Social Services.
Home-Delivered Meals
Available following an assessment to eligible home-bound adults age 60 and older. There are no income-eligibility requirements.
Information and Referral
We are the best source of information about services for older adults, including entitlement programs, health insurance, retirement planning, nursing homes, employment, financial concerns, and home-care options. Listen for us on local radio stations for the latest information about activities and programs for seniors.
Legal Services
An elder law attorney provides help with entitlement programs such as Medicaid, Supplemental Security Income, food stamps, landlord / tenant issues, healthcare proxies and wills. By appointment only.
Nutrition and Education Counseling
- Nutrition Education:
Provided to all congregate and home-delivered meal participants by a registered dietitian.
- Nutrition Counseling:
By a registered dietitian for all meal program participants to help with special diet problems, restrictions and nutrition-related questions.

Senior Identification Program
We Provide photo identification cards. Bring proof of age and residence.
Telephone Reassurance
A daily phone call is made by a volunteer to check on the well-being of home-bound seniors.
